for ONE WAY Magazine's Feb/Mar - Apr/May 2006
Enter To Win (a beautiful
Chicago Blue color, USA-Made Gibson Custom Blueshawk Guitar).
1. To Enter: Complete the official entry form available
at www.onewaymagazine.com/gibson or cut and mail or fax the form in
the Feb/Mar 06 or Apr/May 06 issues of ONE WAY Magazine. Entries must
be received by ONE WAY by June 1st, 2006 to be eligible. Limit one
(1) entry per person per household. Not responsible for lost, late,
or misdirected, entries or entries not received by the deadline date.
2. Prize Winner: The winner will be determined and the prize awarded
in a random drawing from among all eligible entries received. Odds
of winning depend on the number of the eligible entries received.
3. Notification: The winner will be notified by phone and/or email
and will be asked to execute and return an Affidavit of Eligibility/Release
of Liability/Prize Acceptance form within fourteen (14) days of attempted
notification. Non-compliance within this time period may result in
disqualification and the selection of an alternate winner. Return
of any prize/prize notification as undeliverable or a prize unclaimed
within thirty (30) days of notification will result in disqualification
and an alternate winner will be selected.
4. Description of Prize and Approximate Retail Values: One winner
will win a Gibson Custom Blueshawk Guitar (Chicago Blue). The retail
value of the guitar is $1,278. Street price varies. Any additional
costs and expenses incurred and not specifically included in the prize,
including taxes, shipping charges, and/or other incidental expenses,
are the sole responsibility of the winner. Other restrictions may
apply.
5. Eligibility: Open only to residents of the continental United States
who are eighteen (18) years of age or older at the time of entry.
Employees of ONE WAY Magazine and its affiliates, and their parents,
affiliates, subsidiaries, directors, officers and agents, and their
immediate families or those living in the same household, are ineligible
to enter. Only one entry per household. Void where prohibited by law.
All entries not meeting the foregoing requirements are void. ONE WAY
Magazine shall not have any obligation or responsibility with regard
to entries that contain inaccurate information, do not comply with
contest rules, or which are lost, late, incomplete, unintelligible,
misdirected, damaged, or otherwise not received by ONE WAY Magazine,
in whole or in part; or for technical error of any kind; for telephonic,
electronic, hardware, software, network, Internet, or computer malfunctions,
failures, or difficulties.
6. General Conditions: Enter to Win contest subject to federal, state
and local laws and regulations. Winners are responsible for all federal,
state and local taxes. No transfer, substitution or cash redemption
of prize permitted by winner and no more than the listed prize(s)
will be awarded. Sponsor reserves the right to substitute a prize
of equal or greater value if prize(s) is unavailable. Acceptance of
prize constitutes permission to use winners’ names, photographs
and likenesses, for purposes of advertising and promotion without
additional compensation, and for disclosing name of winner on the
website and/or by mail, unless prohibited by law.
7. All entries become the property of sponsor and will not be returned.
Winners agree that the sponsor, its subsidiaries, affiliates, agents
and promotion agencies shall not be liable for injuries, damages,
or losses of any kind resulting from acceptance, possession, or use
of a prize. By participating in this enter-to-win, entrants agree
to be bound by these official rules and the decisions of the judges
which are final in all respects.
8. The name(s) of the prize winner(s), will be available during a
six (6) month period after the end of the contest by sending a self-addressed
stamped envelope to ONE WAY Magazine, 311 N. Robertson Blvd., Suite
